Note:

1. Fill out this form and then give to Ms. Lin with your advisor’s signature. The course will be accepted by the permission of the chief and programme committee in the department. Then, the office will inform the student to get back this form after the permission.
2. The on-line registration of an external-department course without filling out this form will be forbidden.
3. Please show this form when examining the graduation credits, so please carefully keep this form.
